白血病是造血系统出现致命的程序错误，疾病的源头往往是造血干细胞或是早期的祖细胞在发生基因突变后，恶变成了白血病干细胞，它们有两个可怕的特点：一是保持了不断复制的能力，二是完全失去了正常的分化控制，最终的结果就是异常细胞不停增殖，却始终停留在未成熟的幼稚状态，彻底打乱骨髓的正常细胞造血秩序。

不同类型的白血病，发病机制有明显区别：

急性髓系白血病发生在髓系细胞分化的早期阶段，核心问题是细胞卡在了成熟之前，一方面是促进细胞增殖的信号异常增强，另一方面是调控分化的基因受损，让细胞既无法成熟，又能无限分裂；

急性淋巴细胞白血病则发生在淋巴细胞发育的早期，淋巴细胞的发育进程被强行中断，相关基因异常导致淋巴细胞无法成熟，再加上异常增殖信号的驱动，让恶性细胞快速大量增殖；

慢性髓系白血病，病变直接发生在造血干细胞层面，它的发病原因相对单一，主要是染色体易位形成了异常的融合基因，导致酪氨酸激酶持续被激活，虽然细胞还能正常分化，但整个骨髓进入了过度生产的模式，所以在外周血里能看到从幼稚到成熟的各个阶段髓系细胞；

慢性淋巴细胞白血病则有所不同，它发生在成熟B细胞阶段，问题不在于干细胞分化，而是细胞的死亡机制失控，正常情况下本该凋亡的衰老B细胞，因为抗凋亡信号过强得以长期存活并不断堆积，最终形成恶性病变。

随着白血病进展，白血病细胞不光会自身疯狂增殖，还会主动改造骨髓环境，压制正常造血干细胞的功能，让正常造血无法进行。
